Guidelines for Documentation

Psychological Disorders

 

Students who have been diagnosed as having a psychological disorder may be eligible to receive services through the Office of Accessibility and Accommodations if the disability significantly limits major life activities. Examples of psychological disorders are major depression, anxiety disorders, personality disorders, and adjustment disorders.

 

To begin the registration process with our office:
  • SUBMIT the following to A&A:
    1. Documentation (include ONE):

      1. An IEP or 504 plan that is less than 10 years old

      2. OR Psychological Documentation Form completed by a qualified mental health professional   Word     PDF

    2. EIU/A&A Initial Intake Form:   Word   PDF

    3. EIU/A&A Release of Information Form:   Word   PDF

PLEASE NOTE: The review process for requested accommodations may take up to four weeks. The Office of Accessibility and Accommodations will contact students through their EIU email account to schedule an intake meeting.
