Guidelines for Documentation

Deaf or Hard of Hearing

To begin the registration process with our office:

1. SUBMIT the following to A&A:

   1. Documentation (include ONE):

      1. An IEP or 504 plan that is less than 10 years old

      2. OR An audiological report from a licensed audiologist. The report should include the following:

         1. 1. Diagnosis

            2. Prognosis

            3. Limitation

            4. Impact of medical condition on the individual

   2. EIU/A&A Initial Intake Form:   Word   PDF

2. Then SCHEDULE an appointment with our office by email or phone (217) 581-6583.
