All employees required to complete a Statement of Economic Interests based on the criteria identified in the Illinois Governmental Ethics Act (5 ILCS 420/Art. 4A) must also complete the Supplemental Statement of Economic Interest (SSEI). The SSEI is required by Ill. Executive Order 15-09 (January 13, 2015).
Employees who have not received their email with their unique pin should email: eec.ssei@illinois.gov with their name and state agency name (Eastern Illinois University), and the Executive Ethics Commission will follow-up to provide this information.
For all required filers with a designated university email account, instructions containing a website link and a unique pin will be sent by the Executive Ethics Commission (noreply-eec@illinois.gov) the last week of March for electronic filing. Required filers who do not have an email address on file will be sent a paper form to their home mailing address. All forms must be filed electronically or post-marked by May 1st (May 2 for 2022).

Please be sure to fill out the Name, Signature, and Date in the spaces provided at the bottom of the form. Additionally, "Eastern Illinois University" should be written on the line for "Office or position of employment for which this Statement is filed."
