The Student Insurance Fee is automatically included on the tuition and fee statement for students enrolled in an on-campus program with at least 9 hours. The cost for Student Insurance is $109.95 per semester for Fall/Spring and $87.45 for Summer. Student Insurance covers you anywhere off campus, here in the local community, at home, or anywhere you may travel, we do not have a network and provide coverage worldwide. Student Insurance helps pay the deductibles, copays and coinsurance left by your primary. Many students are out of network or have limited coverage when they are away from home.

Student Insurance Enrollment Form - Students are eligible to enroll in Fall 2024 Student Insurance if they are registered for 5 or more hours (any program format) on Census Day (August 30, 2024). Deadline to enroll is August 30, 2024.
Students wishing to purchase coverage must complete the Fall 2024 Student Insurance Enrollment Form by the applicable deadline.
Student Insurance Waiver Form - If you wish to waive the coverage, you will need to complete the Fall 2024 Student Insurance Waiver Form by August 30, 2024. This is for students billed for Fall 2024 Student Insurance. This includes students enrolled in an on-campus program for 9 or more hours for Fall, graduate assistants, or international students enrolled in 3 or more hours.
