Dear EIU Student:
You are invited to participate in a survey research study conducted by Tatjana Magoc, a graduate student in the Department of Kinesiology and Sports Studies, and under the supervision of Dr. Phyllis T. Croisant. The survey is titled:†Physical Activity and Household Food Insecurity as Important Predictors of Health Status in EIU Students.â€
Healthy eating habits are usually related to household food security. Since unhealthy eating habits and lack of physical activity are a major public health concern in the general population, information from this survey will be very helpful to understand the association between levels of physical activity, overweight status, and household food insecurity in college students. The purpose of this study is to test the relationships between the levels of physical activity and overweight status and between household food insecurity and overweight status. You will also be asked to answer demographic questions such as your student classification and type of housing.
If you complete the survey you will be given an option to enter your email address into a drawing for four $25 gift cards to Walmart. However, your email address will not be linked in any way to your survey responses.
Participation in this research study is voluntary and not a requirement or a condition for being the recipient of benefits or services from Eastern Illinois University. Your survey responses will be anonymous and strictly confidential. If you volunteer to participate, you will be asked to respond to series of questions. It will take you approximately 5-10 minutes to complete the survey.
There are no known risks associated with this research, nor will you benefit directly from your participation in this study. This research may help us understand factors that promote or deter physical activity and household food insecurity among Eastern students and design more effective behavioral intervention programs.
You may end your participation at any point during the survey by exiting your web browser. There is no penalty if you withdraw from the study, and you will not lose any benefits to which you are otherwise entitled.
