Please contact Eastern Illinois University Counseling Center at (217)581-3413 if you feel you need the professional guidance. You may withdraw from the survey at anytime if you do not feel comfortable. There are 22 total questions and completion of the survey will approximately take 5-8 minutes.
Your survey responses will be anonymous. The faculty advisor and student will have access to the data collected.
Upon completion of the survey you will be prompted to send an e-mail to be entered into a drawing. The e-mail address was created by the researcher and will only be used for the raffle. Once completed, you will be entered into winning one of three $15 gift cards to Starbucks.
