Dear Student:
You are invited to participate in a research study conducted by Dr. Dejan Magoc from the Department of Health Studies at Eastern Illinois University.
The purpose of this study is to test the relationship between your beliefs about physical activity and your physical activity behaviors. In order for the researcher to examine this topic, you will be asked to answer demographic questions as well.
Participation in this research study is voluntary and not a requirement or a condition for being the recipient of benefits or services from Eastern Illinois University. Your survey responses will be anonymous and strictly confidential. If you volunteer to participate, you will be asked to respond to a series of questions. It will take you approximately 10-15 minutes to complete the survey.
There are no known risks associated with this research, nor will you benefit directly from your participation in this study. This research may help us understand factors that promote or deter physical activity among Eastern students and design more effective behavioral intervention programs.
You may end your participation at any point during the survey by exiting your web browser. There is no penalty if you withdraw from the study, and you will not lose any benefits to which you are otherwise entitled.
