Community Service Registration and Data Form

This MachForm is intended to be completed and submitted by currently active and registered student organizations under Fraternity and Sorority Life.

It is paramount that all FSL Chapter Community Service Events be reported for several reasons: Record of events, logging of hours, contact information, benefitting organization(s), etc. By submitting this MachForm you and your organization provide information that sets you apart from others regarding recognition, news and media coverage, and other opportunities that may benefit your chapter brand to the local and Inter/National Headquarters.
