Faculty can request to have course-related documents audited and remediated by the FDIC.

This form is designed to help faculty request assistance in making course materials accessible to all students. Please note:

1. This service is for course materials ONLY. This includes documents created or adapted by faculty such as syllabi, lecture slides, assignments, and course-specific readings.

2. Faculty are responsible for checking the accessibility of any third-party integrations, links to external websites, and learning media not created by the faculty member.

3. While we strive to make all course materials accessible, it is the faculty member's responsibility to ensure that all aspects of their course, including those outside the scope of this remediation service, meet accessibility standards.
