SICK LEAVE CAN BE REPORTED IN ADVANCE OR ON THE DAY THAT SICK LEAVE IS NEEDED VIA THIS FORM. IN SOME INSTANCES IF YOU CANNOT SUBMIT THIS FORM PLEASE TEXT OR CALL YOUR IMMEDIATE SUPERVISOR PRIOR TO THE START OF THE WORK DAY.
