The Dale and Deb Wolf Outstanding Service Award will be presented to a CHHS faculty member (tenured, tenure-track, or annually contracted) who enthusiastically contributed superior service during Spring 2024 and Fall 2025. The nominee must have completed at least three consecutive academic years of full-time employment at Eastern Illinois University. The nominee must not have received the award within the past five years. Any College of Health and Human Services faculty member or department chair may nominate one eligible colleague for the award. Self-nominations will also be accepted. The following materials should be submitted with each application: *A letter of nomination highlighting how the candidate’s annual service contributions have satisfied (or exceeded) the award criteria. *Two letters of commendation (+ contact information) from individuals familiar with the candidate’s impactful service contributions during the evaluation period. *Note - No other materials will be considered by the Research and Scholarship Committee, who will be reviewing the awards. *APPLICATION DEADLINE* = April 15, 2025 (@ 11:59 pm)
