- Report the injury/illness immediately to your supervisor.
- Call the Tristar Hotline at 1-855-495-1554 IMMEDIATELY after the injury/illness has occurred. If employee is unable to place the call, the supervisor may call.
- Complete the following forms:
- Employee Notice of Injury/Illness Form
- Authorization to Use or Disclose Information
- (Employee Gives to Medical Provider) Initial Workers' Compensation Medical Report
- Have witness/es complete the Witness Report of Injury form.
It is the employee's responsibility to keep the workers' compensation coordinator and his/her supervisor/department informed of his/her status and progress.

Additional Information
Restricted Work Activity
If the injury/illness involves restricted work activity, the employee needs to contact the workers' compensation coordinator and supervisor immediately and forward all appropriate medical documentation. Once this has been performed, the workers' compensation coordinator will contact the supervisor to discuss any modifications or restrictions in regards to work activity. If the modifications or restrictions can be accommodated by the supervisor, the employee shall return to work. If the modifications or restrictions cannot be accommodated, the employee shall be off of work until the modifications or restrictions can be accommodated or a return to work without restrictions is received, whichever occurs first.

Days Away From Work
If the injury/illness involves days away from work, the employee needs to contact the workers' compensation coordinator and supervisor immediately and forward all appropriate medical documentation. If the injury/illness requires the employee to be off of work for more than 14 days, a leave of absence form will need to be completed as well as the appropriate workers' compensation forms. The workers' compensation coordinator can assist the employee with this information and any questions.

Pursuant to Public Act 97-895, the administration of the State Employee Workers' Compensation Program is being transitioned to a third party administrator. A competitive procurement was conducted and an administrator, Tristar Risk Enterprise Management Inc., was selected. Tristar is an experienced third party administrator that provides claims administration services to corporate and governmental entities across the United States.
Effective March 16, 2013, there is a new phone number for reporting all new reports of injury that may result in a workers' compensation claim. It is:

Existing claims will be transitioned to Tristar in the coming months. Each employee who has a claim currently pending will receive an individual letter sometime in the next few months with information regarding their new claim contact.
