A routine procedure required at the time that you leave the campus is the completion of a key clearance sheet. As stated in the graduate assistant regulations, you are required to return all University keys that were issued by Central Stores. Please complete the information below, print the form, obtain the necessary signatures, and present the completed form to the Graduate School Office.

If the form is not returned by (Spring semester: May 15) (Summer term: August 15) (Fall semester: December 15), a hold may be issued against your student record.
